When submitting your questionnaire to OSIP, please follow the instructions below:

To ensure legibility, please type the information into the fillable fields of the questionnaire.

Save the completed questionnaire to your desktop. If you are using a Macintosh computer you must save the form to your desktop first. Reopen the form from your desktop and then begin filling it out. Remember to save the form again.

Print the questionnaire and sign it on the indicated lines.

E-mail, fax or mail the completed signed questionnaire to OSIP.

For those who are not US citizens or green card holders, please also remember to include a copy of your passport information page.
